Program Network

​In-network providers are exclusive to participating Community Health Centers or "Medical Homes."  Your Primary Care Physician (PCP) will be assigned through your Community Health Center clinic.

It is important to see your PCP each year.  Call your assigned Medical Home (Community Health Center) today to schedule an appointment if you haven't seen a doctor in the last year.
