Program Description

​The Primary Care Access Program (PCAP) is a low cost coverage program that provides primary health care services to low-income adults living in Santa Clara County who do not have health insurance.   Enrollees will be assigned a primary care provider and receive care at a participating community health center or "medical home".


  • Adults between 19 and older who live in Santa Clara County;
  • Household income at 400%* or less of the Federal Poverty Level; and
  • Adults not covered by employer health insurance and who are not eligible for full scope Medi-Cal or Covered California subsidies.

To enroll call or walk into your Community Clinic​ to make an appointment with an Enrollment Counselor or as the front desk staff for more information.

PCAP is administrated by VHP Network, a part of the County of Santa Clara.​  ​
