Please call your assigned Medical Home (Community Health Center) within 30 days to report any changes to your personal information, such as:

Address
Family Status
Job Status
Phone Number
Income Status
Marital Status

It is important to carry your PCAP ID card with you in order to get care at your assigned Medical Home (Community Health Center). If you have any questions, call the toll-free PCAP Information Line at 1.888.363.3394 (option 1), Monday through Friday, 9am-5pm, excluding holidays.
